Who Is It For?
Our Outpatient Medication Management Services are for people 18 years of age and older who have a referral from a primary care provider (PCP) or a Behavioral and Mental Health care provider.
We treat most mental health conditions, such as
- Anxiety
- Depression
- Bipolar Disorder
- Schizophrenia
- Anger
Getting Started
Once you’re referred to our services, we’ll give you a call to ensure you are interested. This helps us know that you are aware of the referral and that you are a part of the decision.
If you want to proceed with services, we’ll review your diagnosis. This helps us make sure our services are right for you. If so, we’ll schedule an intake appointment to better understand your condition and needs.
Some patients may already have medication prescribed by a PCP. Using the prescription you already have, we can build on or adjust it to help you reach your goals.
Limited Counseling
Our limited, or as-needed, counseling services connect you to staff who can speak with you about your medication and how you’re doing. This type of counseling occurs as you need it to make sure your medication is still effective.
Managing your medication is a process. We’ll help make adjustments as your condition and goals change, too.
