What Is Plastic and Reconstructive Surgery?
Plastic and reconstructive surgery refers to surgically repairing, restoring, enhancing, or altering the body to improve function or appearance.
Reconstructive surgery focuses on repairing abnormalities or defectsโwhether present at birth, caused by injury, or resulting from diseaseโto restore normal function and structure. Cosmetic plastic surgery focuses on enhancing and improving appearance for individuals who wish to make aesthetic changes that are not medically necessary.

Breast Reconstruction
Breast reconstruction after mastectomy can be performed using implant-based techniques or the patientโs own tissue (autologous reconstruction).
Implant-Based Reconstruction
- One-stage reconstruction: A permanent implant is placed at the time of mastectomy.
- Two-stage reconstruction: A temporary tissue expander is placed at the time of mastectomy. The expander remains in place for approximately 3โ12 months, during which it is gradually filled to stretch the skin. In a second surgery, the expander is removed and replaced with a permanent implant or converted to a tissue-based reconstruction.
Autologous (Tissue-based) Reconstruction
Autologous reconstruction uses the patientโs own tissue to recreate the breast and is also known as โself-transplantโ surgery. Common types include:
- DIEP flap (including stacked DIEP)
- PAP flap
- GAP flap
- LAT flap
- Lateral thigh flap
In these procedures, skin and fatโtypically from the abdomenโare transferred to the breast along with a perforating blood vessel. Using microsurgical techniques, the vessels are carefully connected to ensure healthy blood flow and tissue survival.
Nipple Reconstruction
Nipple reconstruction is typically performed as the final stage of breast reconstruction. The nipple is recreated using the patientโs own skin and may be enhanced with nipple-areola tattooing.
Skin Cancer Reconstruction
Reconstruction is typically offered following the removal of skin cancer. Depending on the size and location of the defect, reconstruction may involve:
- Primary closure
- Skin grafting
- Local tissue rearrangement
- Free flap reconstruction
Head and Neck Reconstruction
Head and neck reconstruction restores form and function, typically following head and neck cancer treatment. Reconstruction may include advanced microvascular techniques, such as free flap reconstruction, to optimize both aesthetic and functional outcomes, such as speech and swallowing.
Perineal Reconstruction
Perineal reconstruction involves surgical restoration of defects in the perineal region, often resulting from cancer, infection, trauma, or congenital conditions. Reconstruction may be performed using local, regional, or free flap techniques, tailored to restore anatomy, function, and quality of life.
Skin Lesion Excision
Benign and malignant skin lesions can be removed in either an outpatient office or operating room setting, depending on the type and complexity of the lesion. If a lesion has not been previously biopsied, a biopsy is typically performed beforehand.
Complex Wound Reconstruction
Reconstruction is available for non-healing wounds, failed prior reconstructions, or complex defects. Treatment options may include skin grafts, local tissue rearrangement, or free flap reconstruction.
Body Contouring After Weight Loss
Patients who have experienced significant weight loss or undergone bariatric surgery may be candidates for body contouring procedures, including:
- Panniculectomy: Removal of excess skin and tissue below the umbilicus (the navel).
- Abdominoplasty (tummy tuck): Removal of excess skin and fat while tightening the abdominal muscles to improve contour and function.
- Brachioplasty: Removal of excess skin and fat from the upper arms.
- Lower body lift: Removal of excess skin and fat from the lower abdomen, hips, buttocks, and outer thighs to create a firmer, more contoured lower body.
Breast Reduction
Breast reduction surgery is designed for women with overly large breasts that cause physical discomfort or functional limitations. This procedure removes excess breast tissue, fat, and skin to reduce breast size, alleviate symptoms, and improve breast shape and proportion.
