Coordinated Care for Complex Cases

Living with a chronic (long-lasting) health condition can be a lot to manage. If you need extra support, or you’re at a higher risk for complications, your care team can connect you to a Registered Nurse (RN) Care Manager.

The RN Care Manager advocates for your health by developing a care plan just for you. As you work toward your health goals, they’ll be there to track progress and make adjustments when needed.

• Develop a personalized, patient-centered care plan
• Decrease avoidable emergency room visits and hospitalizations
• Ensure you have an advocate for needed services and health education
• Manage communication with healthcare providers as you move between care settings
