Employee health injury/exposure reporting is currently being entered into SAFE. The Employee Health First Report of Injury/Exposure will be transitioning from SAFE to the Midas Focus Study where this will be the new data entry and reporting environment for employee injury and/or exposure.
The new online form is available for Kettering Health employees as of January 3, 2023. It has the same questions as the previous form, but with an improved format and flow. This change should make it easier to report work-related injuries and exposures.
How to submit the new Employee First Report of Injury/Exposure online form:
- Click on the Quality & Safety tab on the intranet homepage.
- Click on the Employee First Report of Injury/Exposure hyperlink.
- The form will now open to the Midas Focus Study Entry form. The form instructions are located at the top of the screen. Complete all report sections that are related to your injury/exposure.
- Fill out Sections A, B, and C.
- Sharp Injury/Needlesticks and Mucous Membrane Exposure have additional questions under Sections D and E
- Fill out Section F for blood or infectious disease exposures only.
- After completing the fields above, click Save.
- Click Log Out.

- After clicking Log Out, it will take you to the Bypass Log In screen. Close the browser window.

Important reminder
- Employees must follow up with Employee Health the next business day after the work-related injury, illness, or exposure.
- If you need immediate medical attention for a severe injury, seek treatment through a Kettering Health emergency department.
