Want to learn more about this at Kettering Health?
Heart failure patients navigating their condition face multiple issues, including daily care management and monitoring, as well as risk of hospitalization. To improve the care experience for patients living with heart failure, Kettering Health has partnered with Story Health, a digital health company transforming specialty care delivery. For Kettering Health patients who opt into the program, Story Health will collaborate with Kettering Health providers for virtual, personalized care through medication management, home heart health monitoring, and clinical care team support to help reduce readmission rates and increase provider capacity.
How it works
A licensed and experienced care team from Story Health that proactively engages patients will be available by text, phone or video calls to help patients navigate their condition and the care they need between appointments. Story Health coaches review daily health data supplied by Kettering Health patients to inform any necessary medicine changes, provide education and symptom management, and troubleshoot prescription issues that can arise between patient appointments. Kettering Health will also enroll patients in the Story Health program after being discharged from the hospital, to drive higher engagement in post-discharge follow-up care.
“Heart failure care at Kettering Health combines expertise, cutting-edge technology and specialized care to offer the best experience for our patients. Our partnership with Story Health will extend that care to our patients virtually when they are between appointments to help not only with daily tracking and medicine management in the short term, but ultimately to reduce hospitalizations and improve our heart failure patients’ quality of life,” said Jody Underwood, Kettering Health’s executive director of Population Health.
Improved quality of life
Story Health combines technology and a specialized care team to improve the health and quality of life for people living with cardiovascular health issues. Its care model has delivered superior health outcomes with high levels of engagement—especially for underserved populations.
“Our partnership with Kettering Health ensures patients with heart failure stay engaged, allowing them to receive the highest-quality care. We help the health system reduce readmission rates while also increasing the number of patients they can support,” said Story Health Co-founder and CEO Tom Stanis. “Post-discharge, our experienced care team drives greater engagement from day one, so patients keep up with their care plans.”
Reducing readmission rates is an important priority for health systems and addressing post-discharge barriers can help reduce the risk of complications through early detection and management. Story Health will help Kettering Health take on these challenges by meeting the system’s patients where they are with a compassionate care team who works with each patient to address their needs.
