The Greater Dayton Area Hospital Association is sponsoring the 2024 Community Health Needs Assessment, working in partnership with member hospitals (including Kettering Health), public health departments, and community organizations.

This online survey helps identify the key health needs and concerns of the Greater Dayton community. It asks basic questions about your health, what makes being healthy easy or hard, and how our community is supporting your health.

All Greater Dayton residents are invited to complete this anonymous survey. Completing the survey requires approximately 30 minutes of your time. All who complete the survey will have an opportunity to enter a drawing for one of two $100 Amazon gift cards.

The survey opened April 9 and will remain open for approximately six weeks. Take the survey here.

The final health assessment report will be released in the fall of 2024.
