We are exploring transitioning our Wellness Incentive Program to a Wellness Platform in 2024. A Wellness Platform is a web-based technology that can host a variety of programs used to evaluate, engage, inform, motivate, and incentivize employees based on our wellness strategy.
What does this mean for you?
- We’re not hosting our normal Wellness Incentive Campaign with biometric screenings this year.
- Any employee who earned the incentive for 2023 is automatically eligible for the incentive in 2024. No action necessary.
- Any employee who does not currently have the incentive and wants to earn it for 2024 can still opt in by July 17 by completing a biometric screening and watching the wellness video in HealthStream. Instructions on how to complete these activities are below. Action needed.
- Any employee hired after July 18, 2022 will automatically get the incentive for 2023 and 2024. No action necessary.
If you do not currently have the incentive and want to earn it for 2024, you can complete a biometric screening using one of the two options below and watch a wellness video in HealthStream by July 17. See the chart below for the complete steps to be eligible for the incentive.
| Step One, Option #1: Complete the screening with your primary care provider | 1. Have your physician complete section 2 of the 2023 Physician Screening Form. 2. Send completed (both sections 1 and 2) 2023 Physician Screening Form via email to KetteringHealthPlan@ketteringhealth.org or fax to 937-522-9190. Please note: Only examination and blood work completed between 07/19/2022 – 7/17/2023 will be accepted on the Physician Screening Form. |
| Step One, Option #2: Submit My Chart results | 1. Send completed section 1 of the 2023 Physician Screening Form via email to KetteringHealthPlan@ketteringhealth.org or fax to 937-522-9190. 2. Send My Chart results via email to KetteringHealthPlan@ketteringhealth.org or fax to 937-522-9190. Please note: Only examination and blood work completed between 07/19/2022 – 7/17/2023 will be accepted and all examination and bloodwork listed on the Physician Form needs to be included within the My Chart results. *Height, weight, and blood pressure need to be documented by a physician (hand written results on the physician form will not be accepted). The best way to supply this is to send the first page only of a recent visit summary. Please only send the information requested on the form. |
| Step Two: Watch the wellness video in HealthStream | 1. Log into HealthStream via myHR portal. 2. Search the Catalog for “2023 KH Wellness Incentive Program Education” or “Wellness.” 3. Click on Course Name and click “Enroll.” |

- Engaged PPO plan members: Receive a lower premium
- High Deductible Health Plan (HDHP) members: Receive an employer contribution to your Health Savings Account (HSA) or Flexible Spending Account (FSA), if not eligible for an HSA
